## Access and cost

As of October 2026, the adult online program excludes Alaska, Massachusetts and Rhode Island residents, hospitalized patients and hospice/palliative-facility patients. Head and neck and gynecologic reviews are paused. Pediatric and in-person routes are separate.

As of October 2026, the online fee is $3,000. Most health plans do not cover it; an in-person appointment has separate billing.

Typically 7–14 business days after materials collection, subject to physician availability.
